High School offers On-Campus Sports Physicals Tuesday, June 7

UCHS 2016-Sports-Physical-Flyer_Page_1Physicians will be on campus to provide student-athletes an opportunity to get their annual sports physicals for the 2016 – 17 school year.

  • No appointment necessary!
  • Privacy ensured!
  • Physicians from UCSD’s Sports Medicine Program!

Instructions for completing the sports physical form are available on the UCHS website and on the Centurion Foundation homepage www.uc-centurionfoundation.com.

Cost is $30. We accept cash, credit cards or checks made payable to “Centurion Foundation”. Proceeds to benefit the Centurion Foundation.

Students are NOT required to attend this CF-sponsored event. Students may get their physicals from their medical provider or at any Urgent Care Facility. Please contact them for their pricing information.

UCHS On-Campus Sports Physicals Tuesday, June 7, 2016, 4:30 PM to 7:00 PM, at University City High School, 6949 Genesee Avenue, San Diego, CA 92122, in “The Activity Room”. Students from ALL schools welcome!

Gourmet Food Truck and Street Taco Cart will be on-site for your dining convenience.
